Our parent company, Thrive Tribe, has recently been appointed to deliver a community based cardiovascular disease (CVD) prevention service for Hammersmith & Fulham, Kensington & Chelsea and Westminster in London. Thrive Tribe is putting together a first class team to help us tackle the city’s biggest killer and is recruiting for new members of its team.

Referral Generations and Partnership Officer x 2

As a Referral Generation and Partnerships Officer you work as part of a small team to promote Thrive Tribe’s health improvement services in the triborough. You will work closely with the individual services and Thrive Tribes centralised marketing team to ensure a systematic approach is taken to generating referrals, developing partnerships & networks and ensuring suitable onward referral routes for clients. Both triborough services have challenging targets on health inequalities, BME engagement and delivery expectations.

Healthy Hearts Care Planner x 3

A pivotal role in the team, the Healthy Hearts Care Planners will spearhead the CVD service’s frontline work by delivering clinical assessments and managing a caseload of clients through the services available. This will include using behaviour change techniques to keep clients motivated and engaged as well as being their main point of reference throughout their journey with the service.

Thrive Tribe is looking for a driven individual who can combine impeccable clinical standards with first class communication skills. You will have the flair and determination to motivate clients and help them achieve their health improvement aspirations.

Healthy Hearts Practitioner x 3

This is another key role in the CVD team. Healthy Hearts Practitioners will spearhead the team’s frontline work by delivering an array of exciting community based health improvement interventions including weight management, healthy eating cookery classes and physical activity sessions. This will include working with key community partners and using behaviour change techniques to keep clients motivated and engaged as well as driving positive health outcomes.

Thrive Tribe is on the lookout for a driven individual who can combine a high level of health improvement knowledge with first class communication skills. You will have the flair and personality to motivate clients and help them achieve their health improvement aspirations.
